Typical causes include malformed timestamps, a WebVTT header or dot-style timestamps inside an SRT, broken cue separators, empty cues, invalid numbering, or reversed timing. The destination application can be more strict than a media player.
Reading-speed limits vary by workflow. Practical subtitle tools commonly expose a configurable range around 17–20 characters per second rather than treating one number as universal. Use the specification for your actual delivery.
Mechanical problems such as numbering, sorting, empty-cue removal, some overlap handling and supported tag balancing can be repaired conservatively. Wording and editorial choices should remain for human review.
